JVA Club Insurance provides liability protection for your program and sport accident insurance for your members and staff.

$120/Team, Ages 10-18
$12/Adult Coach, Club Director, Volunteer

$7.00/Volley tot, mini and youth program participant, Ages 3-10
**This is only for in-house groups.  Tams that will play in tournaments outside of their facility will need to be insured at the team rate of $120/team.

$7.00/Participant Tryout Insurance**
**If your club is using JVA Insurance for your club season, simply include an additional team to cover the athletes who participated in your tryouts but were not selected for a team.

How to Sign-Up and Pay for JVA Insurance (for Club Directors/Club Admin):

  • JVA Club Insurance is only available to clubs who have a JVA Club Director member
  • Go to the JVA Member Services page on AllPlayers.com
  • Click on JVA Member Services 2011-2012 Insurance
  • Fill out the quote sheet for the type of insurance you need.  GIVE COMPLETE INFORMATION
  • After you have PAID for your insurance coverage, your Certificates of Insurance will automatically be sent to the name on the form request within 2-3 days.

Each of your participants (players and staff) will need to complete the following forms:

Clubs that are securing JVA Insurance for your club need to click here to get the link for your club's paperwork.  The 2nd tab at the top of the page titled "Member Clubs: Player Forms Submission Links" has each club's link listed.  Direct your families to that link to complete and submit their forms.  Instructions for parents are on the page.  For your convenience, the two forms are combined into one.
