JVA Event Insurance allows teams of any national affiliation (USAV or AAU) to participate in your events.  JVA insurance is only available to clubs who have a JVA Club Director membership. The rates are estimates and may vary depending on the number of events and participants.

$60/team for a Power League or Tournament Series
$30/team for a 3 day tournament
$25/team for a 2 day tournament
$12.50/team for a 1 day tournament

The insurance cost for each of your teams should be included in the entry fee.  Since you will only need to insure those teams that are not JVA insured club teams, you, the Tournament Director, need to discount or refund the portion of the entry fee for those teams equal to the amount of insurance.

 

 To Sign-Up and Pay for JVA Event Insurance:

After you have PAID for your insurance coverage, your Certificates of Insurance will automatically be sent to the name on the form request within 2-3 days.  

 Participant Forms Required for JVA Insured Events

The Head Coach/Team Rep will need to sign a JVA Coaches' Event Sign In Form

prior to his teams participation in a JVA Insured event.  This form lets the Tournament Director know that all the team's required forms (JVA Participant Release of Liability and JVA Release and Medical Form*) have been completed online at AllPlayers.com or that the coach has the forms in his/her possession. Attach the JVA Coaches' Event Sign-in Form to the team roster and retain on file.  You do not need to collect or file any of the insurance forms, only the JVA Coaches' Event Sign-in Form.

What if there is an injury?

Be sure that you have a supply of JVA Incident Reports on hand. If any injury occurs, even if minor:
Secure a copy of the
JVA Release and Medical Form and JVA Participant Release of Liability for your file OR ascertain that the forms exist online at your club’s Allplayers.com JVA Player Form group.  The Head Coach (or whoever witnessed the injury) should complete a JVA Incident Report at the time the injury occurs.  If the injury is serious, email the form to Jenny Hahn at jennyhahn@wi.rr.com.
